Chip N Check Isaac Coast Microchip Vouchers - Registration Form

Isaac Regional Council is collecting personal information you supply on this form in accordance with the Information Privacy Act 2009. Your personal information will be used by Council officers who have been authorised to do so. The information will not be given to any other person or agency unless required by law or unless your permission is sought. Personal information is handled in accordance with the Information Privacy Act 2009.

This program is run in partnership with Sarina Veterinary Surgery in Sarina and Torenbeek Veterinary Clinic in Rockhampton.

Remember, to be eligible for free dog microchipping:

• You must be a resident of the Isaac region with a residential address in the following areas; Carmila, Clairview, Flaggy Rock, St Lawrence, Notch Point, Greenhill and Ilbilbie.
• Your dog must be registered with Isaac Regional Council.
• Dogs that are not yet registered with Council can be registered for free for the current registration period as part of our Chip N Check initiative.*

All fields marked with an asterisk (*) are required.

*Registration with Council needs to be renewed annually. Council is only offering free registration until the next renewal period.
